Approach to the Seriously Ill Patient
EDGE26 Module 1
EDGE26 Module 1: Approach to the Seriously Ill Patient
Latest Course Update: January 2025

Emergency Medicine is a challenging and rewarding field of medical care. Everyday we are faced with medical presentations and diseases we have not seen before. We can easily feel out of our depth as we are challenged to make clinical decisions based on limited information. It can often feel as though we are detectives searching for the clues to solve the mystery of a patient's symptoms.
In this first module we focus on the immediate assessment of the acutely ill or injured patient. How we approach the assessment and initiate treatment may significantly determine a patient's chances of survival, risk for complications, and long-term disability. By the end of the EDGE26 course you will be confident to identify the priorities in assessing the seriously ill patient, recognise immediate life threatening illness and feel comfortable to deliver acute care in the emergency patient within the scope of your clinical skills.

EDGE26 : Recognising and Managing Acute Anaphylaxis
Acute Anaphylaxis
EDGE26: Module 2
EDGE26 Module 2 : Acute Anaphylaxis
Latest Course Update: January 2025

Anaphylaxis is a frightening emergency that may present without warning and requires early recognition and intervention to avoid catastrophic complications. Symptoms develop rapidly often within several minutes, (but may develop within seconds to as long as an hour), after contact with the allergen. Urgent treatment is required to prevent progression to cardiac arrest.
In this module we explore the range of presentations for anaphylaxis and emphasise the awareness we need to carry with us at all times for this life threatening but readily treatable condition. It is certainly true that the early recognition and treatment of acute anaphylaxis can save a life and the following topic is one of significant importance to Emergency Medicine.

Acute Chest Pain
EDGE26: Module 3
EDGE26 Module 3 : Acute Chest Pain
Latest Course Update: January 2025
In this module we explore the clinical approach the acute chest pain focusing our attention on searching out the clinical clues to life threatening diseases such as acute coronary syndrome or pulmonary embolism. This is critical on two accounts : Firstly we wish to avoid missing serious disease with a risk that the patient may be sent home. Secondly we know that in acute coronary syndrome the earlier we identify disease the earlier we can initiate interventions reduce morbidity and mortality.
Time is critical in assessing the patient with chest pain and we need to approach assessment systematically to identify / exclude serious disease. In this module we take on this challenge and outline how to approach to these patients in a way that will allow early recognition of serious disease and trigger appropriate interventions that reduce morbidity and mortality.

EDGE26 Module 4
EDGE26 Module 4 : ECG Interpretation (1) Arrhythmias
Latest Course Update: January 2025
Skills in ECG interpretation are central to the assessment and management of patients presenting with chest pain, syncope, palpitations and shortness of breath. Arrhythmias are common and although in many cases the ECG diagnosis is relatively straight forward however this is not always the case and a systematic approach to reading the ECG is important to avoid misdiagnosis and risk of incorrect treatment.
In this module we explore some basic principles with respect to the ECG interpretation, outline a systematic approach to reading an ECG and then focus on the ECG diagnosis of cardiac arrhythmias. The approach is intended to be highly practical and able to be immediately applied to the clinical care of the emergency patient.

EDGE26 Module 5 : ECG Interpretation (2) Myocardial Ischaemia
Latest Course Update: January 2025
The ECG plays a central role in the evaluation of the patient with acute chest pain. The ST/T wave component of the ECG waveform reflects repolarisation of the ventricle. Abnormalities of the ST segment commonly result from abnormal depolarisation of the ventricle and injury or inflammation of myocardial tissue due to myocardial ischaemia or acute pericarditis.
In this module we focus on the patterns of ECG changes associated with myocardial ischaemia arguably one of the most interesting and challenging areas of ECG Interpretation !

Latest Course Update: January 2025
Sepsis is a life-threatening condition that arises when the body's response to infection injures its own tissues and organs. It is a common cause for life threatening deterioration and is associated with a high mortality rate !
In this module we consider the clinical presentation of sepsis and the current approach to the management of the patient where the emphasis is on correcting or preventing circulatory shock and reducing the risk widespread organ dysfunction characteristic of severe sepsis. Early recognition and treatment is critical to survival as the presence of sepsis substantially increases the risk for ICU care or death, early recognition and treatment are critical.

EDGE26 Module 7
EDGE26 Module 7 : Acute Headache
Latest Course Update: January 2025
Acute headache is a common presenting problem. Although often due to benign causes, headache may signal serious or life threatening disease such as meningitis or subarachnoid haemorrhage. The first priority is to search for factors indicating a serious underlying cause and at the same time provide relief to the patient for the symptoms of headache.
In this module we explore the clinical approach the acute headache and focus on identifying the Red Flags that indicate a significant risk for serious disease and a need to investigate the patient. We also consider the most effective approach to the pharmacological management of the patient who presents with severe migraine headache.

EDGE26 Module 8
EDGE26 Module 8 : Acute Pulmonary Oedema and CPAP
Latest Course Update: January 2025

Respiratory distress is a common acute presenting symptom. While the Primary Survey will direct us to apply oxygen as part of the initial management of the patient presenting with severe respiratory distress this in fact is only the starting point for care for the patient with severe respiratory distress where we need to move quickly to initiating definitive treatment of the underlying condition.
In this module we focus on patients presenting with the clinical features suggestive for acute pulmonary oedema and consider the treatment options for this common life threatening presentation. In particular we explore the use of noninvasive ventilation for treating severe respiratory distress. We hope you find this topic a useful introduction to this fascinating and exciting area of emergency medicine.

EDGE26 Module 9
EDGE26 Module 9 : Blood Gas Interpretation
Latest Course Update: January 2025

Blood Gases are important in the assessment of the patient with disordered acid-base balance. In this module we explore some of the basic principles for reading blood gases focusing on identifying acidosis and alkalosis and distinguishing metabolic from respiratory causes for an abnormal blood gas result.
In contrast to other modules where a Case Simulation is used to teach the topic, this module uses a series of short videos that outline the core principles. After each video is a short exercise that aims to enable you to apply the principles discussed in the video to the interpretation of blood gases. Blood gas interpretation is a very practical skill and the exercises allow you to practice recognising the patterns associated with acid base disorders.

EDGE26 Module 10
EDGE26 Module 10 : Severe Agitation
Latest Update: January 2025

The patient with severe agitation can be extremely confronting to the clinician presenting not only major issues with respect to assessment and management but may also represent a risk to the safety of staff and patients. The differential diagnosis is broad and may be as simple as “a bottle of vodka” or as sinister as meningitis and a careful assessment of both their physical and mental health is essential. The immediate priority is to reduce their risk of harm and identify serious medical causes such as head injury, hypoglycaemia, infection, drug intoxication or withdrawal.
In this module we explore the immediate priorities in the severely agitated patient, introduce the "Mental Health Primary Survey" and examine the procedure and pharmacological options for managing severe agitation.

EDGE26 Module 11
EDGE26 Module 11 : Acute Poisoning
Latest Course Update: January 2025

The poisoned patient is one of the most complex emergency presentations and requires rapid assessment and stabilisation. Immediate life-threatening complications may occur (often with little or no warning) and require advanced skills in resuscitation (including airway management and vascular access).
In this module we consider the approach to the poisoned patient focusing on the "Risk Assessment" and techniques for minimising toxicity by reducing absorption, enhancing drug elimination from the body or reversing the toxic effects of the poison. Poisoning is a huge area of emergency medicine but fortunately in most circumstances we can apply our Primary Survey and Resuscitation skills and seek advice on the next step in management from databases or contacting our local Poison information service.

Envenomation: Snake and Red Back Spider Bite
EDGE26 Module 12
EDGE26 Module 12 : Envenomation: Snake and Spider Bite
Latest Course Update: January 2025
In this module we will explore the assessment and management of patients presenting with red back spider bite and snake bite.
Although suspected snakebite is a relatively common ED presentation during the spring and summer months, it is only rarely is associated with the clinical findings of severe envenomation. Management is time critical and begins with appropriate first aid of the snakebite followed by clinical and laboratory assessment of the patient for features indicative of envenomation.
While Red Back Spider bite is the most common cause of significant envenomation presenting to emergency care, in contrast to snake bite the venom can be slow acting and does not cause life threatening envenomation. Considerable controversy exists with respect to the use of antivenom in red back spider bite with an evolving change in clinical practice.

Latest Course Update: January 2025

Acute abdominal pain is a common cause of serious illness. As the population ages, emergency care will be increasingly confronted with elderly patients presenting with acute abdominal pain. It is vital to have a sound approach to the assessment, investigation and management of these patients as presentations with serious pathology can be subtle and easily missed in this population.
In this module we examine the red flags for serious illness to the elderly patient with acute abdominal pain, identify the life threatening conditions that may present in this group and consider the role of investigations to assist identifying or ruling out serious disease.
This topic assist the clinician to develop a structured and informed approach to assessment that will reduce the risk of missing serious disease in this population.

EDGE26 Module 14 : First Trimester Bleeding
Latest Course Update: January 2025

In this module we explore the assessment, differential diagnosis and management of this PV bleeding in early pregnancy.
Bleeding and / or pelvic pain in the first trimester are common emergency presentations in the pregnant patient. For most patients the symptom will be the result of a threatened or complete miscarriage however a cautious approach should be routinely applied to the assessment aiming to rule out the major life threats including ectopic pregnancy, septic abortion and incomplete miscarriage.
Approaching the pregnant patient can be a little disconcerting for those with little or no obstetric experience. This module has been written to foster a great understanding of the complications that may occur in the pregnant patient and build confidence in assessment and management of these special patients.

EDGE26 Module 15 : Recognising and Managing the Seriously Ill Child
Latest Course Update: January 2025

It is easy to miss the seriously ill child who presents with subtle clinical findings, or feel uncomfortable when required to manage the seriously ill child. The young child with acute illness presents major challenges to the clinician due to incomplete history (eg the infant who is unable to report symptoms), difficulties with examination (eg the crying or uncooperative child) and the subtle features of serious disease that may be easily overlooked or missed.
In this module we focus on the assessment and immediate management priorities in the seriously ill child with a focus on the clues (or Red Flags) in the history and clinical findings on examination that should alert us to the presence of serious illness.

EDGE26 Module 17
EDGE26 Module 17 : Severe Asthma in a Child
Latest Course Update: January 2025

The child with severe life threatening asthma is arguably one of the most frightening paediatric emergencies experiences to manage as a clinician. As failure to recognise the severity of the attack and under-treatment of severe illness are major contributing factors to mortality resulting from asthma it is critical to evaluate the child carefully and identify severe or life threatening asthma requiring urgent bronchodilator therapy and supportive care.
In this module we focus on the clinical clues in history (red flags) that signal a child at high risk for a life threatening asthma and explore the currently recommended approach to managing severe and life threatening asthma in a child drawing on recent research that has informed National Clinical Guidelines.

Emergency Management of Major Trauma
EDGE 26 Module 18
EDGE26 Module 18 : Major Trauma
Latest Course Update: January 2025

Although the initial management of the trauma patient, follows a structure very similar to the management of other life threatening presentations, the assessment and management of the trauma patient is complicated by the possibility of multiple life threats due to concomitant injuries.
There is a very real concern that some serious injuries may not be identified during the initial assessment. This mandates a comprehensive (head to toe) examination of the patient, use of a wide variety of investigations and continuing review of the patient's clinical condition to identify evolving clinical signs of serious injury. In this module we explore the immediate resuscitation priorities, detailed assessment and cervical spine clearance in the patient presenting following major trauma.

Assessment and Emergency Management of Burns
EDGE26 Module19
EDGE26 Module 19 : Emergency Management of Burn Injury
Latest Course Update: January 2025

There are many priorities in managing the patient with burns and these pose a challenge to the clinician in deciding how to approach management. Particular attention should be placed on assessing the airway for evidence of inhalational burn injury that may progress to complete airway obstruction. Fluid resuscitation requires careful thought to ensure that not only is shock identified and treated but also that consideration is given to the continuing loss of fluid from the burn injury.
In this module we explore the immediate priorities in the management of burn injury including cooling of the injury, local burn management, burn dressings and referral, analgesia, tetanus prophylaxis and giving consideration to the possibility of other trauma related injuries and toxic gas exposure such a carbon monoxide poisoning.

EDGE26 Module 20
EDGE26 Module 20 : Acute Pain Management
Latest Course Update: January 2025

Although the relief of acute pain is the most common reason that patients seek emergency care, research studies have documented that acute pain is frequently poorly managed in the ED setting. There is often a complete failure to provide any form of analgesia in a timely manner and even when analgesia is provided it is often ineffective due to either poor drug selection, inappropriate route of administration or inadequate dosing.
This module draws on our current understanding of the pathophysiology of pain and takes a clinical approach that focuses on the assessment of acute pain and explores strategies for managing pain, selection of agents and principles for continuing pain management.

EDGE26 Module 22 : Lower Limb Orthopaedic Injury
Latest Course Update: January 2025

In this topic we continue our journey of discovery through bony and soft tissue limb injury - this time focusing on injuries to the lower limb.
We consider severe orthopaedic injuries such as fractured neck of femur and fractured femoral shaft, limb threatening injuries such as fractured tibia and fibula with the significant risk of compartment syndrome and the common soft tissue injuries around the ankle including "ankle sprain" and ruptured achilles tendon noting that these apparently less severe injuries unless diagnosed and treated correctly can lead to long term disability for the patient.

EDGE26: ENT Emergencies
Introduction to ENT Emergencies
EDGE26 Module 24
EDGE26 Module 24 : ENT Emergencies
Latest Course Update: January 2025

There are a wide range of ENT emergencies and in this module we explore three of the most difficult ENT emergencies; epistaxis, severe tonsillitis / peritonsillar abscess (quinsy) and bells palsy.
Severe tonsillitis is a common presentation and after ensuring there is no threat to the airway may be effectively managed medically in most cases with significant relief of the patient's symptoms. In the patient with bells palsy the focus is often excluding other causes for facial palsy, prevention of eye injury and considering the role of pharmacological management.
Epistaxis can range from the trivial self-limited nose bleed to bleeding that results in life-threatening airway obstruction and circulatory shock. In this final module in the EDGE24 course our case simulation explores in detail the aetiology, assessment, investigation and management of epistaxis with the aim of making you more confident to approach and manage the patient presenting with epistaxis.

EDGE26 Module 23 : Eye Emergencies
Latest Course Update: January 2025

Eye emergencies are common presentations to acute care. The causes may vary from minor eye irritation through to severe eye threatening disease or injury. Unless we have had the opportunity to undertake training in this topic the assessment and management of eye emergencies can be very challenging and perhaps a little bit frightening ("What am I missing ?").
In this module we set about building the foundation to assessing and treating eye emergencies in clinical practice. We begin by exploring the assessment of the patient presenting with an eye related problem highlighting the red flags that assist to identify patients at risk for serious eye disease and discuss in detail the emergency examination of the eye. In the Case Simulation we examine the differential diagnosis for the red eye and follow a number of clinical cases of patients presenting with the complaint of a (painful) red eye.
